Getting An Account

Apply for Access

You must fill out and return two forms in order to gain access to FMW. The first is a request for access. The other is an Access to Information sheet. They can be found below:

• FMW Access Request Form
• Campus' Access to Information Statement

Fill out these forms completely and send them to Insuh Pak(ikpak@ucsc.edu)
